For access to personal data in accordance with the Personal Data (Privacy) Ordinance ('PD(P)O'), please complete the Data Access Request Form  which is specified by the Privacy Commissioner for Personal Data.

For obtaining photcopy of personal data in accordance with PD(P)O, a photocopy charge set by the Treasury will be levied .  The prevailing charge is HK$1.00 per photocopy for photocopying black and white on A3 or A4 size paper.
