Through the exercise of 'Re-engineering Community Support Services for Elders' launched in April 2003, all Home Help Teams (HHTs) (except one located in Tung Chung, Lantau Island) had been upgraded to Integrated Home Care Services Teams to provide various kinds of care and support services to the frail elders, people with disabilities and other people with special needs. The remaining HHT continues to provide service to elders aged 60 or above, people with disabilities, individuals and families living in Tung Chung area who are incapable of looking after themselves. The scope and frequency of services provided will be based on the assessment on individual cases.
