Aftercare Service for Dischargees of Halfway House

Service Description
To facilitate dischargees of half-way houses to reintegrate into the community and to help them adjust to their daily living as well as to their social and working environments.

Eligibility for the Service
Ex-mentally ill persons successfully discharged from halfway houses.

Application Procedures
Referrals are self-arranged between half-way houses and agency-based aftercare workers

Fees and Charges
The service is provided free-of-charge.
