DAY CARE SERVICE FOR PERSONS WITH SEVERE DISABILITIES

Objective

Day Care Service for Persons with Severe Disabilities provides centre-based day care for persons with severe disabilities. Its objective is to promote care in the community by strengthening the caring capability of families or carers through provision of regular day care including nursing, rehabilitation, social and personal care services so as to enhance opportunities of persons with severe disabilities to continue living in the community.

Target Group

Persons with severe intellectual disability or severe physical disability aged between 15 to 59

Nature of Service

The services provided include:
	day care including meal;
	nursing care and intensive personal care including assistance and activities of daily living;
	maintenance programmes on basic living skills; and
	regular activities to meet service users' social and recreational needs, and to enable them to maintain contact with the community.

Operation Hours

From 9:00 a.m. to 5:00 p.m. or from 9:00 a.m. to 6:00 p.m., Monday to Friday

Fee and Charges
	$988 per month, lunch included
	$55 per day or $60 per day, lunch included

Referrals can be made by social workers to the service unit direct
