Service Description Home-based Training and Support (HBTS) service aims to provide home-based training and establish support network for people with disabilities and their families through utilization and integration of rehabilitation services and community resources.
The following range of services should be provided:
| (a) |
training and support services for people with disabilities and their families including the provision of appropriate Individual Training and Support Plans; |
| (b) |
occupational therapy service for people with disabilities; |
| (c) |
network support service for people with disabilities and their families; and |
| (d) |
Day Activity Centre (DAC) service. | |

Target group and eligibility criteria

Application Procedures Except for DAC service which is from the Central Referral System for Rehabilitation Services, operating NGOs receive referrals/ applications from the following sources:

direct referrals from school social workers, medical social workers, family caseworkers and staff of rehabilitation service units; or |
 |
direct applications from people with disabilities or their families |

Fees and Charges $51 per Occupational Therapist's visit $21 per annum for family membership fee for network support service |
