Pilot Scheme on Home Care Service for Persons with Severe Disabilities
Service Objectives Pilot Scheme on Home Care Service for Persons with Severe Disabilities (the Scheme), a 3-year pilot project from March 2011 to February 2014, provides persons with severe physical disability / mental disability who are on the waiting lists for subvented residential care services with a package of home-based support services to meet their care, nursing care and rehabilitation training needs. It aims at relieving the stress of their family members / carers and improving their quality of life.
Target Groups Persons with severe disabilities who are living in Tuen Mun District / Kwai Tsing District / Kwun Tong District / Wong Tai Sin District and on the waiting lists for Hostel for Severely Mentally Handicapped Persons, Hostel for Severely Physically Handicapped Persons or Care and Attention Home for Severely Disabled Persons.
Programme content

The Scheme provides a package of home-based support services, including : |

(i) |
personal care; |
|
(ii) |
escort service; |
|
(iii) |
occupational therapist / physiotherapist rehabilitation training service; and |
|
(iv) |
nursing care service. |

Carer support service. |
Source of Referrals Referral will be made by Rehabilitation and Medical Social Services Branch of Social Welfare Department.
