Service Description
It provides assistance to patients with visceral disability or chronic illness to establish a supporting network in the community and to live a meaningful life through the provision of on-going support in the form of educational and training programmes, social networking and social support services. |

Eligibility for the Service
 |
people with chronic illnesses (such as cardiac disease, renal failure, diabetes and haemophilia); |

|
families, relatives and friends of people with chronic illnesses; or |
| |
|
|
the general public. | |

Public can approach community rehabilitation network directly for enquiry on application procedures. |

Fees and Charges
The membership fee is $21 per annum. |
