Service Description Specialized Home-based Training and Support (SHBTS) service provides home-based professional therapy services to physically handicapped persons and mentally handicapped persons to support them living in the community and perform the core services of Home-based Training and Support.
The following range of services should be provided:

specialized occupational therapy and physiotherapy services include:

physical training, mobilization and strengthening |
| - |
sensory motor training |
| - |
advice and practical training on self-care activities in real life situation |
| - |
fabrication, prescription and training on use of rehabilitation equipment/devices |
| - |
recommendations on environmental design and home alteration |
| - |
pre-vocational assessment and training |
| - |
care-giver education | |
| (b) |
training and support services for people with disabilities and their families including the provision of appropriate Individual Training and Support Plans; |
| (c) |
network support service for people with disabilities and their families; and |
| (d) |
Day Activity Centre (DAC) service.

not receiving any therapeutic treatment from hospitals and rehabilitation centers |

priority should be given to people with severe disabilities not receiving any rehabilitation service | |

Training and support service

people with disablities aged 15 and above |
| - |
living in the community and not receiving any rehabilitation services (Limited quota (not more than 10%) could be provided to sheltered workers who are assessed and referred by social workers for training in their activities of daily living with a view to continue the living in the community) |

priority should be given to severely disabled persons | |

Network support service

people with disabilities aged 15 and above and their families |

priority should be given to people with severe disabilities and who are not receiving any rehabilitation services | |

Application Procedures Except for DAC service which is from the Central Referral System for Rehabilitation Services, operating NGO receives referrals / applications from the following sources:

direct referrals from school social workers, medical social workers, family caseworkers and staff of rehabilitation service units; or |
 |
direct applications from people with disabilities or their families | |

Fees and Charges $51 per Occupational Therapist's visit $21 per annum for family membership fee for network support service
