Integrated Home Care Services

Integrated Home Care Sevices (IHCS) provide a range of community support services to the elders, people with disabilities and needy families living in the community with a pool of experienced and professionally trained staff, and via a network of service units in the community with its collaboration and support. The Integrated Home Care Services Teams (IHCSTs) provide care and support to the target service users according to their individual needs and actualize the concepts of 'Ageing in Place' and 'Continuum of Care' to enable the service users to continue living in the community.

Application and Enquiry

*	Frail cases:
	Service users can approach respective Integrated Family Service Centres/Integrated Services Centres, Medical Social Service Units, District Elderly Community Centres, Neighbourhood Elderly Centres and Social Centres for the Elderly or rehabilitation services units for referrals and arrangement of Standardized Care Need Assessment for Elderly Services. For further enquiry on the Standardized Care Need Assessment for Elderly Services for frail cases, please contact the Standardized Care Need Assessment Management Offices (Elderly Services) [SCNAMO(ES)s]. *	Ordinary cases:
	Service Users can approach respective Integrated Family Service Centres/Integrated Services Centres, Medical Social Service Units , District Elderly community Centres, Neighbourhood Elderly Centres and Social Centres for the Elderly or rehabilitation services units for referrals or IHCSTs direct for application.
