Home-based Training and Support Service

Service Description
Home-based Training and Support (HBTS) service aims to provide home-based training and establish support network for people with disabilities and their families through utilization and integration of rehabilitation services and community resources.

The following range of services should be provided:

(a)

training and support services for people with disabilities and their families including the provision of appropriate Individual Training and Support Plans;

(b) occupational therapy service for people with disabilities;
(c) network support service for people with disabilities and their families; and
(d) Day Activity Centre (DAC) service.
   
* Target group and eligibility criteria
* Training and support service
- people with disabilities persons aged 15 and above
-

living in the community and not receiving any rehabilitation services
(Limited quota (not more than 10%) could be provided to sheltered workers who are assessed and referred by social workers for training in their activities of daily living with a view to continue the living in the community)

- priority should be given to severely disabled persons
* Occupational therapy service
- people with disabilities aged 15 and above
- living in the community
- not receiving any therapeutic treatment from hospitals and rehabilitation centres
- priority should be given to people with severe disabilities not receiving any rehabilitation service
* Network support service
- people with disabilities aged 15 and above and their families

priority should be given to people with severe disabilities and who are not receiving any rehabilitation services

* DAC service
- severely mentally handicapped persons aged 15 and above
- waitlisted for DAC or DAC cum Hostel service

Application Procedures
Except for DAC service which is from the Central Referral System for Rehabilitation Services, operating NGOs receive referrals/ applications from the following sources:

*

direct referrals from school social workers, medical social workers, family caseworkers and staff of rehabilitation service units; or

* direct applications from people with disabilities or their families

* Fees and Charges
$51 per Occupational Therapist's visit
$21 per annum for family membership fee for network support service
