Programme Evaluation  

	Upon completion of the Tier I Programme and Tier II Programme in each year, participating NGOs are required to submit an evaluation report for each participating secondary level of each participating school.  The related documents should be submitted in both hard and soft copies, the required documents include: Tier I Evaluation Report         MS Word Doc Form A Total Count Sheet     MS Excel Doc Form B Total Count Sheet     MS Excel Doc Tier II Evaluation Report        MS Word Doc  Form C Total Count Sheet    MS Excel Doc
	The evaluation report should be countersigned by the school principal concerned and submitted to the Hong Kong Jockey Club Charities Trust, together with the NGO's Claim Form and Income and Expenditure Statement of each secondary level of each participating school (a copy of the statement should be provided to the school principal concerned).
