The Social Welfare Department implements case management service in the Home Care Service for Persons with Severe Disabilities, Integrated Support Service for Persons with Severe Physical Disabilities, and District Support Centre for Persons with Disabilities.  Case managers provide one-stop support services for persons with disabilities and their carers so as to co-ordinate and arrange appropriate rehabilitation and welfare services for them throughout their different life stages.  Handbook on Case Management Service (Chinese version only) can be downloaded here.
