  Service Objectives 
 

District Support Centre for Persons with Disabilities (DSC), by adopting a district-based approach, provides one-stop community support services for persons with disabilities and their family members / carers.  It aims at enhancing the domestic living and community living skills for persons with disabilities so as to facilitate their integration into the community through the provision of a range of community support services.  It also provides training and support services to the family members / carers of persons with disabilities so as to strengthen their caring capacity and to relieve their stress.

  Target Groups
 
 1. Persons with disabilities living in the community;
 2. Family members / carers of persons with disabilities; and
 3. Community at large.
   
  Programme Content
 

DSC would meet the needs of persons with disabilities by providing a range of services and activities, including :

  
 1.

Individual or group training programmes and support services for persons with disabilities to enhance their domestic living and community living skills;

 2.

Social, recreational and personal development programmes for persons with disabilities;

 3.

Training programmes / educational courses / workshops for family members / carers of persons with disabilities to enhance their caring capacities;

 4.

Supportive services to family members / carers of persons with disabilities to relieve their stress;

 5. Community education;
 
 6. Drop-in service for persons with disabilities; and
 7.

Information on community resources and referral services for appropriate welfare services

 8.

Provision of case management service including conducting need assessment, formulation of case plan and rendering counseling service to the persons with disabilities and their families and to bridging up community resources.

   
Source of Referrals

Persons with disabilities and their family members / carers can directly approach the DSCs for services.  The application is free of charge.

 
Fees and Charges
Membership fee is $21 per annum.
