$d Background and Objective

The Community Care Fund (CCF) launched the programme of “Special Care Subsidy for the Severely Disabled” (the Programme) in September 2011, which aims at providing severely disabled persons who are living in the community and not receiving the Comprehensive Social Security Assistance with a special care subsidy for assisting them to purchase care goods, services or for other purposes related to nursing care. The Programme was extended in November 2012, 2013, 2014, 2015 and 2016 respectively. CCF now extends the Programme again to provide eligible severely disabled persons with a subsidy for a maximum of 24 months. The Programme is administered by the Social Welfare Department (SWD).

$d Eligibility Criteria
An applicant should meet ALL of the following criteria:
  • as at 31 July 2018, he/she is aged below 60 and receiving Higher Disability Allowance (HDA) under the Social Security Allowance (SSA) Scheme; and
  • lives in the community(note 1) and fulfills relevant monthly household income requirement [i.e. the monthly household income of the applicant and his/her family members living in the same premises in Hong Kong does not exceed the income limit applicable to the corresponding household size (Annex); while no asset limit is imposed.]
Note 1:

It refers to those not receiving care in residential institutions subsidised by the Government (including subsidised places in subvented/contract homes and residential care homes under various bought place schemes) or any public hospitals and institutions under the Hospital Authority, or boarding in special schools under the Education Bureau.

 $d  Applicant

Persons who meet the eligibility criteria can apply for the subsidy. If an applicant is aged below 18 or has been medically certified to be unfit to make a statement, the application should be made by the guardian/appointee currently approved by SWD to receive HDA under the SSA Scheme on his/her behalf.

$d Application Procedures
  • Letters (enclosing application forms) will be issued by SWD to notify the persons who may be eligible.
  • Applicants/guardians/appointees are required to submit the completed application forms together with all necessary documents set out below (relevant income proof is not required when submitting an application) by post or by hand to the CCF Team of SWD on or before 28 February 2019:
    • a copy of the applicant’s identification document; and
    • a copy of the guardian/appointee’s Hong Kong identity card (applicable only to an applicant who is aged below 18 or has been medically certified to be unfit to make a statement, but this is not required in the case of an application made by a social worker of SWD as the applicant’s guardian/appointee).
$d Vetting of Application and Payment of Subsidy
  • SWD will process the applications according to the information submitted by the applicants/guardians/appointees. After completion of vetting, notification letters will be issued by SWD to the persons concerned and the subsidy will be paid to the eligible persons. SWD will conduct random checks in the course of processing the application or arranging payment of subsidy, and the applicant/guardian/appointee will be required to provide detailed information of his/her income and other relevant information for verification.
  • Individuals receiving HDA and aged below 60 during the period between 31 July 2018 and 30 July 2020 are eligible for the subsidy, which will be granted to them for a maximum of 24 months. If a beneficiary is no longer receiving HDA or has reached 60 during the period concerned, the subsidy will be payable to him/her until the month in which the relevant change takes place(Note 2).
  • SWD will determine the rate of subsidy an eligible individual is entitled to for the whole period based on the average monthly household income for the three months prior to submission of application (i.e. not exceeding 100%, or over 100% to 125%, or over 125% to 150% of the relevant Median Monthly Domestic Household Income). The rate of subsidy is set as full grant ($2,000 per month), three-quarters grant ($1,500 per month) or half grant ($1,000 per month). Please refer to Annex for calculation of the rate of subsidy.
  • Subsidy will be paid on a quarterly basis. From the end of February 2019 onwards, SWD will deposit the first quarter’s subsidy directly into the bank account of eligible persons in which they receive HDA, and will thereafter regularly verify the subsidy that an eligible individual is entitled to and arrange payment accordingly on a quarterly basis.
Note 2:

The subsidy will be terminated starting from the subsequent calendar month when the beneficiary becomes ineligible during the period between 31 July 2018 and 30 July 2020. If he/she becomes eligible for the Programme again (i.e. resume receiving HDA and is still aged below 60) during the concerned period, he/she may approach the CCF Team of SWD for re-application on or before 31 October 2020. SWD will arrange payment of the subsidy he/she is entitled to based on his/her relevant circumstances during the period mentioned above.

$d Responsibilities of the Applicant/Guardian/Appointee

Information provided by the applicants/guardians/appointees must be true, correct and complete. In case of any change of the relevant information, they should report the change to the CCF Team of SWD as soon as possible. They must provide detailed information for verification when SWD conducts random checks on their cases. Any subsidy certified as overpayment will have to be paid back.
