District Services and Community Care Teams – Pilot Scheme on Supporting Elderly and Carers

Description

In March 2024, the Social Welfare Department has implemented a 12-month "District Services and Community Care Teams - Pilot Scheme on Supporting Elderly and Carers" (the Pilot Scheme). Taking Tsuen Wan and Southern District as pilot points, District Services and Community Care Teams (Care Teams) are engaged to help identify households of singleton / doubleton elderly and carers of elderly persons / persons with disabilities in need through visits or contacts, and refer these cases to social welfare service units for follow-up.  The Care Teams will also assist in referring elderly and persons with disabilities in need to install and use the emergency alarm system (EAS).  

Target Groups

  • Singleton elderly households;
  • Doubleton elderly households;
  • Carers of elderly; and
  • Carers of persons with disabilities.

Programme Content

  • To identify, through visits or contacts, households of singleton / doubleton elderly and carers of elderly / persons with disabilities who are in need;
  • Follow-up caring services – to provide continual care and concern to the households of singleton / doubleton elderly and carers of elderly / persons with disabilities who are in need, through visits, telephone contacts, face-to-face contacts, activities, etc.;
  • Follow-up support services – to introduce information about social welfare services or community resources; assist in referring needy cases to apply for EAS or assistive equipment (e.g. wheelchair, walking stick, etc.); fill in application forms for referring needy cases to social welfare service units; organise support programmes and sharing sessions, etc.; and
  • Assist in referring elderly and persons with disabilities in need to install and use the EAS.
