Day Care Service for Persons with Severe Disabilities

Description

Day Care Service for Persons with Severe Disabilities (DC/SD) provides centre-based day care for persons with severe disabilities. Its objective is to promote care in the community by strengthening the caring capability of families or carers through provision of regular day care including nursing, rehabilitation, social and personal care services so as to enhance opportunities of persons with severe disabilities to continue living in the community.

Target Service Users

The target service users are persons aged 15 to 59- 

  • With severe intellectual disability or severe physical disability; and
  • With mild or moderate intellectual disability who are assessed to have confirmed ageing conditions by designated District Support Centre for Persons with Disabilities.

Service Nature

DC/SD provide the following services-

  • Day care including meal;
  • Nursing care and intensive personal care including assistance and activities of daily living;
  • Maintenance programmes on basic living skills; and
  • Regular activities to meet service users’ social and recreational needs.

 Operation Hours

DC/SD is attached to different Care and Attention Homes for Severely Disabled Persons (C&A/SD), District Support Centres for Persons with Disabilities (DSC) and Community Rehabilitation Day Centres (CRDC).  Operation hours are as follows-

Fees and Charges

The service fees for DC/SD are-

  • $1,002 per month (including lunch); and
  • For daily service fee, please contact with the concerned unit for details.

Referrals

  • Referrals can be made by social workers to the service unit direct.
