Day Care Service for Persons with Severe Disabilities

Description

To provide centre-based day care for persons with severe disabilities. Its objective is to promote care in the community by strengthening the caring capability of families or carers through provision of regular day care including nursing, rehabilitation, social and personal care services so as to enhance opportunities of persons with severe disabilities to continue living in the community.

Target Group

  • Persons with severe intellectual disability or severe physical disability aged between 15 and 59.
  • Persons aged 15 to 59 with mild or moderate intellectual disability who are assessed to have confirmed ageing conditions by designated DSC can apply for DC/SD.

Scope of Services

  • day care including meal;
  • nursing care and intensive personal care including assistance and activities of daily living;
  • maintenance programmes on basic living skills; and
  • regular activities to meet service users' social and recreational needs.

Operation Hours for Day Care Service for Persons with Severe Disabilities attached to the following facilities

Day care service for persons with severe disabilities is attached to different Care and Attention Homes for Severely Disabled Persons (C&A/SD), District Support Centres for Persons with Disabilities (DSC) and Community Rehabilitation Day Centres (CRDC).  Operation hours are as follows:
  • C&A/SD : 9:00 a.m. - 5:00 p.m., Monday to Friday;
  • DSC : 9:00 a.m. - 6:00 p.m., Monday to Friday;
  • CRDC : 8:00 a.m. - 6:00 p.m., Monday to Friday.

Fees and Charges

  • $1,002 per month (including lunch);
  • For daily service fee, please contact with the concerned unit for details.
