The Department of Health, in collaboration withChai Wan Baptist Church Community Health Centre implement the 'Elderly Health Assessment Pilot Programme”. This Programme provides voluntary, protocol-based and subsidized health assessment to elderly people aged 70 or above who hold valid Hong Kong Identity Cards. Priority will be given to hard-to-reach elderly people who live alone and have not had health assessment and regular medical follow-up by health services before.
The Programme comprises three parts: 1. baseline health assessment,
2. one to two follow-up consultations and 3. health promotion; which includes physical examination, health assessment, blood and other tests, ECG, stool specimens,gynecological examination, explanation of reports, doctor’s follow-up, individualcare plans, referrals to specialists, health talks and workshops.
Elderly people who fulfill the criteria are invited toparticipate in the Programme.
