Solicitation of Signed Authorisation Forms

Introduction

Solicitation of Signed Authorisation Forms mainly involves face-to-face solicitation of signed authorisation forms, in paper or electronic form, where the donation payment is made on a one-off and/or regular basis, by autopay via credit cards or bank accounts, etc.

Eligibility Criteria

To be eligible for applying a public subscription permit for organising solicitation of signed authorisation forms in public places, the applicant shall meet the eligibility criteria [please refer to Part B “Eligibility Criteria” of the “Explanatory Notes for Application for Public Subscription Permit (Solicitation of Signed Authorisation Forms)”].

Application Procedures

Application for a public subscription permit for organising solicitation of signed authorisation forms in public places must be subimitted from 1st to 31st January of each year [applicable to application for latter half of the same year (i.e. from 1st July to 31st December)], or from 1st to 31st July of each year [applicable to application for former half (i.e. from 1st January to 30th June) or the whole of next year]. The applicant is required to submit the following relevant forms and documents:

(Note: Applicant organisation and/or beneficiary organisation(s), if applicable, is required to submit its annual reports or newsletters or reports of charitable service record as endorsed by its Chairperson or Head to support its track record of charitable activities for the past three years. (Note: The permittees should draw reference to this sample to report all the monies from public subscriptions and all sort of expenses in relation to the fund-raising activities covered by the public subscription permit.)

Submission of Application

Organisations must apply for Public Subscription Permit (Solicitation of Signed Authorisation Forms) for the whole or the former half of next year or the latter half of the same year at the designated application timeframe. For organisations which intend to conduct solicitation of signed authorisation forms for the whole next year, they only need to submit the application form in the former half and tick both boxes of “Application for Former Half Year” and “Application for Latter Half Year” at item (1) on the form. If there are any changes in meeting the eligibility criteria (e.g. registration, tax-exemption status, Articles of Association or constitution, etc.) after the Public Subscription Permit is issued, the organisation concerned has to inform SWD immediately.

The completed application form (regardless of public streets or public places other than public streets) together with all the required documents as listed in the application form, should reach the following designated address on or before the above mentioned period.
