NGOs are required to submit the following reports through the Service Performance Management Information System (SPMIS) e-Submission System: 

• Review Reports on Remuneration Packages for Staff in the Top Three Tiers
• Service Performance Monitoring System (SPMS) self-assessment reports
• Internal Service Audit Report
• Disclosure of Annual Financial Reports (AFRs)
• Funding and Service Agreement-related Activities Annual Statement (with effect from 2024-25)

•  Schedule for the Utilisation of Reserve in Holding Account (HA) for 2024-25 and the Plan of Utilisation of HA Reserve for 2025-26

• Schedule for Funding and Service Agreement (FSA) services / FSA-related activities supported by Other Funds or Donations for Designated Purposes (with effect from 2024-25)

• Special Incident Report
  NGOs are required to report to SWD the special incidents in respect of the subvented service unit(s) not more than three calendar days (including public holidays) following the incidents by completing the Special Incident Report form.

• Significant Incident Report 
  NGOs should submit the Significant Incident Report to SWD as soon as possible when there are significant incidents involving public interest, such as governance crisis, financial management problems or other issues that may have impact on service or manpower deployment.
