District Support Centre for Persons with Disabilities

Description

To provide one-stop community support services for persons with disabilities and their family members / carers , to enhance the domestic living and community living skills for persons with disabilities so as to facilitate their integration into the community.  It also provides training and support services to the family members / carers of persons with disabilities so as to strengthen their caring capacity and to relieve their stress.

Target Groups

  1. Persons with disabilities living in the community1;
  2. Family members / carers of persons with disabilities; and
  3. School Leavers of Special Schools.

Programme Content

  1. Individual or group training programmes and support services for persons with disabilities to enhance their domestic living and community living skills;
  2. Social, recreational and personal development programmes for persons with disabilities;
  3. Training programmes / educational courses / workshops for family members / carers of persons with disabilities to enhance their caring capacities;
  4. Supportive services to family members / carers of persons with disabilities to relieve their stress;
  5. Community education and promotion of centre services;
  6. Drop-in service for persons with disabilities; 
  7. Dissemination of information on community resources and referral services for appropriate welfare services; 
  8. Case management service including conducting need assessment, formulation of case plan and rendering counseling service and to bridging up community resources for the persons with disabilities and their families; and
  9. Designated Team for Special School Leavers attached to the centre provides early intervention and extra care and support services to the special school leavers and their carers with a view to smooth transition from school to community living for special school leavers. (The service of Designated Team will be launched in April 2024.)

Application Procedure

Directly approach the DSCs for services or referrals of Cases are mainly made by special school social workers to the Designated Team.

Fees and Charges

Membership fee is $21 per annum.
