Name of Programme Jockey Club Discharge Support and Rehabilitation Programme

Objectives To facilitate rehabilitation of people with dementia (PWD) discharged from hospital for ageing in place;
To help families to provide good dementia care to the care recipients to prevent recurrent hospitalisation or premature institutionalisation due to improper care
Contents A 12-week transitional care programme inclusive of:
●Up to 4 weeks of respite service with cognitive activities and personal care,
●Up to 8 weeks of day care centre service with cognitive activities,
●Up to 2 therapeutic treatments each week,
●Home assessment with suggestions for a safe and dementia friendly environment by occupational therapist
●Multidisciplinary support by a team of nurse, physiotherapist, occupational therapist, and social worker,
●Case management provided by nurse or social worker, and
●Support to the families of care recipients and a long time care plan on departure from the programme.
Target Participants ●PWD due to be discharged from acute hospitalisation in Hong Kong,
●Those planning to resume ageing in place with their family caregivers or domestic helpers on departure from the programme, and
●Those able to adapt to the social environment setting of the programme.
Fee ●Pre-payment of HKD18,500 being 50% of the standard fee for 4 weeks of respite and 8 weeks of day care centre service; the remaining 50% sponsored by The Hong Kong Jockey Club Charities Trust;
●If use of the respite service is less than expected, or the care recipient needs to depart from the programme earlier, the pre-payment will be refunded on a pro-rata basis.

Name of Programme:Support Services for the Needy Carers / elderly persons

Venue:North District
Objectives:To help the carers to relieve their caring burden by providing tangible support services.
Contents:Household cleaning service
(i) Household cleaning service which is provided by trained care worker
(ii) Three hours (one-off) household cleaning service
(iii) Insurance premium is covered

Escorting service(i) Escorting service which is provided by trained care worker to attend medical appointment
(ii) Four hours (one-off) escorting service
(iii) Service user is required to pay the transportation fee
(including taxi fare if required)
(iv) Insurance premium is covered
Target Participants:(i) Registered carers and the elderly persons of the centre who are not receiving subsidised community care services.
(ii) Priority will be given to the needy carers with financial difficulty.
(iii) Referral from social worker is required

Objectives:To explore the mutual understandings between older adults and foreign domestic helpers (FDHs) for better psychosocial wellbeing of both parties.
Contents:To train foreign domestic helpers (FDHs) with communication skills, elder caring skills and skills in developing life story book with older adults.
