Standardised Care Need Assessment Mechanism for Elderly Services & Central Waiting List

The Standardised Care Need Assessment Mechanism for Elderly Services (SCNAMES) and the Central Waiting List for Subsidised Long Term Care Services (CWL) provide assessment and registration for subsidised long term care (LTC) services at single entry point so as to facilitate elderly persons in making applications, waitlisting and allocation of such services. 

Applications for the following subsidised LTC services are required to undergo standardised care need assessment, with a view to assessing applicants’ needs and matching them with service according to the assessment results.

The SCNAMES covers applications for the following subsidised LTC services:

  1. Community Care Services (CCS):
  2. Residential Care Services (RCS):

SWD has set up five multi-disciplinary Standardised Care Need Assessment Management Offices (Elderly Services) [SCNAMO(ES)s] in the regions of Hong Kong, East Kowloon, West Kowloon, New Territories East and New Territories West to implement the SCNAMES.

New applicants

The Central Waiting List accepts requests for long term care services only on a referral basis. Applicants or persons acting on behalf of the applicants may approach the following referring offices for raising the request:

  1. District Elderly Community Centres (DECCs) / Neighbourhood Elderly Centres (NECs);
  2. Medical Social Services Units (MSSUs);
  3. Integrated Family Service Centres (IFSCs) / Integrated Services Centres (ISCs); and
  4. Others service units e.g. Family and Child Protective Service Units (FCPSU).

Application procedures

1. Raise a Request: An elderly person or his/her family raises a request for subsidised LTC services to a responsible/ referring worker of a Medical Social Services Unit, an Integrated Family Service Centre or an elderly service unit in the district.
2. Initial Screening: Responsible/referring worker conducts initial screening and refers the elderly person for arrangement of assessment as appropriate.
3. Conduct Assessment: Assessor carries out the assessment through home visit and face-to-face interview.
4. Explain the Assessment Result: Responsible/referring worker explains to the elderly person the assessment result and service matched.
5. Formulate a Care Plan: Responsible/referring worker draws up a care plan for the elderly person and assists him/her to apply for suitable service.

Existing users of subsidised long term care services

Existing users of subsidised long term care services may approach their request with their referring offices / service providers if they find the services they are receiving no longer meeting their care needs. The referring offices / service providers acting as responsible workers may make appropriate referrals in respect of their users' needs:

Service users of subsidised places under the Enhanced Bought Place Scheme (EBPS) / Day Care Centres for the Elderly Service users of other subsidised long term care services
  1. District Elderly Community Centres (DECCs) / Neighbourhood Elderly Centres (NECs);
  2. Medical Social Services Units (MSSUs);
  3. Integrated Family Service Centres (IFSCs) / Integrated Services Centres (ISCs); and
  4. Others service units e.g. Family and Child Protective Service Units (FCPSU).
  1. Subvented Residential Care Homes for the Elderly (RCHEs) / Contract Homes;
  2. Integrated Homes Care Services Teams (IHCS) / Enhanced Home and Community Care Services (EHCCS); and
  3. Day Care Units for the Elderly attached to DECCs/RCHEs.
